You will be asked sixty-six questions about the ministry of your church. You'll answer each question on a scale that goes from "Totally Disagree" to "Totally Agree". To fill in the survey just click on the button that best corresponds to your answer.

 

The questions are sorted around twenty-two indicators of church vitality. Each indicator has three questions. Additionally, the questions are grouped into five sections:

 

• Community of Faith

• Believing

• Belonging

• Behaving

• Leading

 

As you consider each question, prayerfully answer them as you think about your congregation. There might be some areas you are not sure about - that's OK. Try to answer each question the best you can.

 

To begin the survey, enter the passcode that was provided to you and click on "Continue".

 

When you have finished the last question you will see:

 

"Thanks for Taking the Survey."

 

You can then log off the site.
